Release Form
Writing for Publication Class Student Release FormWhile the purpose of a class like ours is to be published, how high school students are published is the decision of the parents, teachers, and administrators, so you are requested to sign a release form stating that your child is allowed to be published in the formats described. Generally, first and last names have been used in distributed print versions of publications, while first name and middle initial have been the standard online. It will be your child’s responsibility to publish work with the identification you choose, though it will be checked. This form indicates your understanding that students are responsible for their own content, and this content may be viewed by many individuals, including those with the right to alter or delete. Therefore, students and parents may choose not to publish some work or to have some removed. The class will require a percentage of all work to be published, so audience does play a role that students must take into consideration. Mr. Jaeger would love to publish all of the works his students create, but since the sites are associated with his position at Menomonee Falls High School and viewable by anyone with internet access, he must limit the public posts to assignments (not blogging). While what is appropriate online is certainly a matter of opinion, please keep in mind that content deemed inappropriate may require explanation and will still likely result in it no longer being published. Mr. Jaeger will also have to explain or defend some aspect of the content or assignment, and he would rather not have to do this. Please circle one for each category below:
